Nursing Model of Care

The Nursing Model of Care at the Greater Hazleton Health Alliance is a team-oriented approach. The "Know Your Patient Model of Care" is a representation of nursing practice in which a qualified professional nurse is responsible and accountable for the nursing care of an assigned group of patients.

The RN Charge Nurse is responsible for making the assignments for the oncoming shift and the assignments are comprised of RN/RN or RN/LPN teams who deliver care to an assigned group of patients in order to allow the nurses the opportunity to work closely with a specific group of patients so that they can get to "know the patients" who are assigned to their care.

RNs receive report on their assigned patients whenever they come on shift and do regular rounds on their patients in order to assure the proper level of care and tend to any patient needs. Bed assignments are made by the Nursing Supervisor on duty at the time of admission.
